How so? It just means its not a matched set, meaning the body will be swapped, but hands and feet will be from a different body and therefore may have seams, gaps, etc.  If your looking for perfection your going to have to learn Bodyslide to make sure the outfits match the body.

That's a problem with the skyrim engine when trying to play an animation on a ragdoll.  You should configure DA to use only Bleedout.

Well since some of those errors are DLC content I'd say you need to LOOT your load order, or a very broken/incompatible mod installed somewhere.

Was your character playing the cover self animation? If so that's a known issue with the animation having sound enabled.

You sure the schlong wasn't changing position just before a sexlab stage advance, that's normally when I see the schlong adjust itself.
