How do I link something with different text?

Okay so I want to link my blog on my signature but I have no idea how to place it how I want it. I can put the link for it but what I want to do is have a word you can click and it takes you there instead of having to paste the whole link. I saw this:

 

 

It asks for an id. I don't know how that works can anyone help me out? I looked to see if I had some king of ID but I don't see anything.
